Applying for Membership
Introduction
To become a member of REDEEM® HealthShare you will need to complete an application. REDEEM HealthShare charges a non-refundable $100 application fee, paid upon submitting the membership application form.
You can complete the application form independently, or if you prefer, you can call 877-641-1579 or email, and someone from Member Care can assist you. Completing the application will take about 10 to 15 minutes.
You won’t send your initial month’s share upon completing the application, rather you’ll send it on the first day of the month of your membership start date that you chose.
Information
For a smooth and efficient application process, have the following information ready before calling or going online:
- Contact information
- Email address
Note: Write down the email and password used on the first page of the application so that if you cannot complete the application, you can use the Member Sign In option on the homepage when you return to complete the application. - Phone number
- Email address
- Identification information
- ID numbers from a state-issued driver's license or a state-issued ID card
Important: When you create your profile, type your name as it appears on your state issued driver’s license or ID card. - Social Security numbers
- ID numbers from a state-issued driver's license or a state-issued ID card
- Personal data for all potential members
- Name
- Birthdate
- Medical history
