Applying for Membership

REDEEM® HealthShare Ministry charges a non-refundable $100 application fee, paid upon submitting the membership application form.

For a smooth and efficient application process, have the following information ready before calling or going online:

• Personal data for all potential members
  • Name
  • Birthdate
  • Medical history
• Potential primary and participating spouse's identification
  • Social Security numbers
  • Identification numbers from a state-issued driver's license or a state-issued ID card
• Potential primary member contact information
  • Email address
  • Phone number
