Register for a Group Sponsorship

Introduction

REDEEM® HealthShare prefers that as the Authorized Representative of your Group Sponsorship that you complete the Sponsorship Registration because as the Authorized Representative, you must sign and date the Declarations and Disclosures.

However, if as the Sponsor, you choose to register and someone else will be the Authorized Representative, you must use his or her information and then share the login and password with him or her.

Procedure

As the Authorized Representative, you must complete the Sponsorship Registration application. To complete this task, follow these steps:

  1. Click the Sponsorship Registration link to access the application. The Sponsor Center screen will open.
  2. At the lower right of the screen, click Begin Registration. The Register a Sponsoring Entity screen will open.
  3. Type the information in all the required fields:
  4. Click The Designate an Authorized Representative screen will open.

  5. Type the Authorized Representative’s information in all the required fields.
  6. Select the ReCAPTCHA checkbox.
  7. Click Next. The Get a Quote (Forecast Costs) screen will open.
    • If you want to get a quote, see Get a Quote for a Sponsorship Group, otherwise continue with the next step.
    • The member information you use in getting a quote automatically adds those members to your Sponsorship Group.
  8. To bypass the quote option and continue the registration process, at the lower right of the screen, click Skip. The Create Sponsored Group screen will open, and you can:
    • Add a group within the Sponsorship.
    • Add a Group Administrator. This is beneficial if you do not manage the group payments.Create a Sponsored Group screen
  9. In the Sponsored Group Name field, type a group name or select Same as Sponsor.
  10. If you want to add a Group Administrator, then:
    • Select Yes. Go to step 11.
    • Select No. Go to step 12.
  11. Type the Group Administrator’s information in the required fields.
  12. Select Same as Sponsor.
  13. Click Next. The Add Payment Options screen will open.
  14. Type the account information in the required fields.
  15. Click Next. The Declarations & Disclosures screen will open.
  16. Read and confirm your agreement with the declarations.
  17. Select the checkbox beside each declaration to agree.
  18. Type your name as your signature and the date.
  19. To complete the registration, click Submit.

As the Sponsor, you must sign the declarations and disclosures for the Sponsorship Group. In addition, each individual group member must also sign the declarations and disclosures when he or she applies.

Tips

The Sponsorship login is for administrative purposes. If your Authorized Representative will also be an individual member of the group, he or she will need a separate login for his or her personal REDEEM membership.
