Send a Message Using the Contact Us Form

Introduction

Thank you for your membership or your interest in membership with REDEEM® HealthShare Ministry. You are welcome to send suggestions, give feedback, and ask questions.

If you are a current member and you experience a technical problem within the Member Center, use this form as well to send a ticket to the service desk.

Procedure

If you want to send a note or report a technical problem, follow these steps:

1. Select Contact Us.
2. From the Get In Touch With Us page, type your name and your email address in the appropriate fields.
3. From the CONTACT US CATEGORY field, select the dropdown arrow and choose the option that best describes your request type.
4. Type your question, suggestion, feedback, or technical problem in both the required QUESTION and ADDITIONAL DETAILS fields.
5. Select the I Agree check box. 
   Note: You may need to complete a reCAPTCHA to verify that the request is not from a robot.
6. Click Submit Your Question. A thank you message displays.
