To continue participation with REDEEM™ HealthShare Ministry and to reaffirm your commitment to the program's faith-based and lifestyle requirements, you must renew your membership annually by:
- submitting the Membership Continuation Agreement form confirming that all members in the household continue to meet membership requirements.
- requesting that your church leader, pastor, elder, or small group leader sign and submit the Church Leader Verification (CLV) form confirming all members in the household adhere to the faith and lifestyle standards outlined in Guidelines – I.A. Statement of Faith and Lifestyle Requirements.
- providing any updated personal information, including changes in your family status or contact information.
As a member of REDEEM, reading and understanding the Sharing Guidelines is important.
